Housing and Human Services is hiring for a High-Fidelity Wraparound Care Coordination Supervisor to join the Partnerships, Contracts, and Services (PCS) Division within the Prevention Services team.
This position leads a team of High-Fidelity Wraparound facilitators that work to engage youth and families that need support towards specific goals and stability. The supervisor coaches’ staff on adhering to the evidence-based model and structure to ensure positive outcomes and fidelity for Boulder County families. A solid understanding of the services available in the community is critical and how to help families and staff access them, as well as identifying new or needed services. The position must foster an open, accepting attitude towards all clients and professionals and create an engaging and trusting culture that is founded upon racial equity. This is a full-time, benefited position. The work schedule is Monday - Friday 8:00 am - 5:00 pm. Occasional evening and weekend work may be required. This is a hybrid position with the ability to work remotely and from the 1333 Iris Avenue, Boulder office in addition to some in office and community meetings required. Under FLSA guidelines, this position is exempt. New employees receive an 80-hour bank (Download PDF reader) of vacation at the time of hire, in addition to 8 hours of both vacation and medical leave accruals each month.
Boulder County requires its employees to reside in the state of Colorado as of the first day of work.
Hiring Salary Range: $ 61,680.00 - $75,258.00 Annually Tentative Hiring Timeline:

Examples of Duties
- Provide supervision of High-Fidelity Wraparound Facilitators and Support Partners.
- Monitor the delivery of High-Fidelity Wraparound according to national standards.
- Demonstrate the skills required to obtain Wraparound credentials as a High-Fidelity Wraparound Coach and Supervisor, in accordance with state level standards.
- Utilize various types of data to improve practice of the High-Fidelity Wraparound team, including ensuring regular Fidelity measurement tool implementation, data collection that support program delivery, implantation, and evaluation for Boulder County High Fidelity Wraparound.
- Develop and implement procedures to enhance and increase internal and external efficiency and customer care.
- Develop and train staff in utilizing all required documentation tools.
- Participate in various committees, community programs, and boards related to program activities and responsibilities.
- Maintain organizational relationship at the local and state level with Colorado’s High Fidelity Wraparound System of Care community.
- Educate youth, families, professionals, natural supports, and the community about the Boulder County High Fidelity Wraparound program
- Manage screening and intake of community referrals to best meet the needs of Boulder County youth and families.
- Respond to and manage the ongoing training and coaching needs of the High-Fidelity Wraparound Team, reviewing credentials in conjunction with performance evaluations, and recommending staff for continuing education
- At times, the High-Fidelity Wraparound Supervisor may be required to take on a Facilitation caseload, and therefore must be willing to perform as a High-Fidelity Wraparound Facilitator duties at times. These potential duties may include:
- Coordinate and facilitate Wraparound team meetings made up of youth, their families, professionals, and natural supports
- Develop integrated, individualized plans of care with teams that are based on the identified strengths and needs of the youth and family and reflect the best possible fit with their culture, values, beliefs, and family vision
- Interview youth, their families, professionals, and natural supports to identify strengths, needs, family culture, and vision
- Create a comprehensive crisis prevention plan with the team which includes creative strategies to assist the youth to remain safely in the community
- Performs related work as required
- May be reassigned in emergency situations

EXPERIENCE OR EDUCATION:
Boulder County is looking for well qualified candidates to fill our positions. Any combination of relevant education and experience is encouraged. In this position, we are looking for a minimum of 8 years of combined education and experience in social services, human services, juvenile justice services, facilitation skills and coaching experience or related field, including one year in a supervisory capacity
